Duovir-N® manufacturer is Cipla. Its active chemical compounds are Lamivudine (3TC), Zidovudine (ZDV) that are synthetic guanosine nucleoside reverse transcriptase inhibitors (NRTIs), and Nevirapine (NVP) – non-nucleoside reverse transcriptase inhibitor (NNRTI).


- treatment of human immunodeficiency virus (HIV-1) infection, as soon as patient has been stabilized on the maintenance regimen of Nevirapine 200 mg twice a day, and have demonstrated adequate tolerability to Nevirapine.

MOA (mechanism of action):

Intracellularly Zidovudine is phosphorylated to its active 5-triphosphate metabolite, zidovudine triphosphate (ZDV-TP) and Lamivudine is phosphorylated to its active 5-triphosphate metabolite, lamivudine triphosphate (3TC-TP). Their mode of action is to inhibit reverse transciptase (RT) via DNA chain termination after incorporation of the nucleotide analogue. Nevirapine directly inhibits RT by blocking the RNA-dependent and DNA-dependent DNA polymerase activities. Nevirapine has no effect on HIV-2 RT and eukaryotic DNA polymerases (such as human DNA polymerases α, β, γ or δ).


The recommended oral dose is 1 tablet (Lamivudine 150 mg, Zidovudine 300 mg, Nevirapine 200 mg) twice daily.

Duovir-N® should not be initiated in adult females with CD4^^ cell counts greater than 250 cells/mm^3^ or in adult males with CD4 cell counts greater than 400 cells/mm^3^ unless the benefit outweighs the risk.

Duovir-N® should not be given to patients who have just initiated therapy with Nevirapine. This is because an initial lead-in dosing of 200 mg Nevirapine once daily for 2 weeks, alongside the standard doses of Lamivudine + Zidovudine twice daily is recommended.

The 14-day lead-in period with Nevirapine 200 mg daily dosing has been demonstrated to reduce the frequency of rash. If rash continues for longer than 14 day lead-in period, do not increase dose to 200 mg twice daily. The 200 mg once daily dosing regimen should not be continued for longer than 28 days, after which an alternative regimen should be determined.


HIV can be transmitted through sexual contact with HIV positive partner, blood (e.g., sharing needles, handling blood instruments, receiving blood transfusions, blood products, or organ/tissue transplants that were infected with HIV), childbirth, toiletries (razors and toothbrushes). Therefore, both HIV-positive and HIV-negative individuals are advised to have any kind of sex using a latex or polyurethane condom, avoid sharing needles or other things like toothbrushes or razors. It is important to discuss with doctor the precautions needed to avoid infecting other people as well as take precautions not to get infected.

Women who are HIV-positive must not breastfeed because HIV infection can be passed on to the baby in breast milk.

Take special care if you have moderate or severe liver disease, you have ever had liver disease, including hepatitis B or C, you are seriously overweight (especially if you are a woman), you have severe kidney disease.

Consult doctor before taking if you are taking phenytoin (for treating epilepsy), methadone (as a heroin substitute). Zidovudine speeds up the removal of methadone from the body. If you are taking methadone, you will be checked for any withdrawal symptoms. Your methadone dose may need to be re-elevated.

Some people who take Duovir-N® may experience:

- Hematologic toxicity, including neutropenia and severe anemia (low red blood cell count) and neutropenia/leucopenia (low white blood cell count), which is associated with Zidovudine.

- Lactic acidosis (a build-up of lactic acid in the body), together with an enlarged liver. Symptoms are deep, rapid breathing, drowsiness, and non-specific symptoms such as nausea, vomiting and stomach pain.

- Immune Reconstitution Syndrome (immune response to fight infections that may have been hidden in the body).

- Severe acute exacerbations of hepatitis B have been reported in patients who are co-infected with hepatitis b virus (HBV) and HIV-1 and have discontinued Lamivudine.

- Prolonged use of Zidovudine has been associated with symptomatic myopathy.

- Bone problems – osteonecrosis (death of bone tissue).

Missing a dose

If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose of Duovir-N® to compensate the missed dose.


If an overdose occurs and you are not feeling well, stop taking the medication immediately. You should seek emergency medical attention or contact your healthcare provider immediately.


Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F) and away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Side effects

The most common and uncommon side effects associated with Duovir-N® are:

  • headache
  • malaise and fatigue
  • fever or chills
  • nausea
  • diarrhea
  • nausea and vomiting
  • anorexia and/or decreased appetite
  • abdominal pain
  • abdominal cramps
  • dyspepsia
  • neuropathy
  • insomnia and other sleep disorders
  • dizziness
  • depressive disorders
  • nasal signs and symptoms
  • cough
  • skin rashes
  • musculoskeletal pain
  • myalgia
  • arthralgia

Side effects occurrence does not only depend on the medication you are taking, but also on your overall health and other factors.


Duovir-N® is contraindicated in patients who are allergic to Zidovudine, Lamivudine, Nevirapine or any of the other ingredients of Duovir-N®.

Do not combine Duovir-N® with other lamivudine-, zidovudine-, or emtricitabine-containing products.

Do not take Duovir-N® if you have or have had very low red blood cell count (severe anemia) or very low white blood cell count (neutropenia), kidney disease or liver disease (such as hepatitis), peripheral neuropathy, pancreatitis, hepatitis B or C and, hemophilia, diabetes mellitus, erectile dysfunction.

Do not combine Duovir-N® with such medications astemizole or terfenadine (for allergy); amiodarone, bepridil, dronedarone, encainide, flecainide, propafenone, quinidine (used for irregular heartbeats); dihydroergotamine, ergotamine (for migraines); ergonovine, methylergonovine (for stopping excessive bleeding); clorazepate, diazepam, estazolam, flurazepam, triazolam or oral midazolam (help to fall asleep); clozapine, pimozide, (psychiatric drugs); quetiapine (for schizophrenia, bipolar disorder and major depressive disorder); lurasidone (for depression); ranolazine (for chronic chest pain); pethidine, piroxicam, propoxyphene (used to relieve pain); cisapride (for stomach conditions); rifabutin (for certain infections); voriconazole (for fungal infections); simvastatin, lovastatin (for lowering blood cholesterol); alfuzosin (for enlarged prostate); fusidic acid (for bacterial infections); PDE5 inhibitos (sildenafil, avanafil); colchicine (for gout), products containing St John’s wort, tavudine, ribavirin, valproic acid, fluconazole, probenecid, pentamidine, dapsone, pyrimethamine, cotrimoxazole, amphotericin, flucytosine, ganciclovir, valganciclovir, interferon, vincristine, vinblastine, and doxorubicin.

Consult your doctor before taking Duovir-N® if you are pregnant or plan to have a baby.
